I understand that the LJFL may require a medical waiver issued by a doctor for some conditions..

I understand that the LJFL does not supply any type of medical insurance for league participants.

I understand that  I am responsible for any medical exspenses incurred from injuries while participating in the LJFL.

I understand that my child is to be accompanied by someone 18 years of age or older at all league functions.

I understand that no refunds will be issued after July 20th. and the refunds requested before this date will be charged a $25.00 fee.

I agree to participate in a league fundraiser by purchasing 20 raffle tickets in May 2010

I agree to attend the League's mandatory parent meeting

I agree that a player  must attend league weigh-in to be placed on a team
